The Website, Other Websites, the App, and our Communications are collectively referred to as the “Platform”.

Medical Emergencies
OUR WEBSITES AND ONLINE SERVICES ARE NOT INTENDED TO BE USED TO ADDRESS MEDICAL EMERGENCIES. IF YOU ARE EXPERIENCING AN EMERGENCY OR URGENT MEDICAL PROBLEM, GO TO THE NEAREST EMERGENCY ROOM OR CALL 911 IMMEDIATELY.
Online Services
- Access to test results, medication lists, and medical record summaries.
- Online bill pay.
- Video visits for online medical care.
- Appointment scheduling with medical professionals or testing locations.
To provide these services, Anew needs to access and you grant permission to access the following options on your device:
- Location
- Camera
- Microphone
- Files, Media, and External Storage
- Bluetooth and Wi-Fi
- Phone Number
- IMEI
You may also share your Personal Data with us via third-party services such as Apple Health (iOS devices) or Google Fit (Android devices).
Creating an Online Account
Our online services will prompt you to create an online account when necessary to use the online service. Follow each prompt and provide the requested information to set up your account. For the best user experience, regularly update your web browser.
Accessing Your Child’s Medical Record Online
If you have a child between the ages of newborn and 17 years, you may access your child’s medical record online by:
1. Completing the Child Proxy Access Form and providing a copy of your legal representative documentation (if applicable) to Anew registration personnel.
2. Setting up an account online through the Platform.
Parental or legal representative access to the child's portal will automatically terminate when the child turns 18 years old.
## Responsibilities and Conduct
By using our Platform, you agree not to:
- Use our websites and services in a manner that could disable, overburden, or impair our websites and services or interfere with any other party's use and enjoyment of our Platform.
- Seek to obtain access to any materials or information through hacking or other unauthorized means.
- Submit material to the Platform that is false, defamatory, unlawfully threatening, or unlawfully harassing.
- Transmit materials through our Platform that contain viruses, Trojan horses, worms, time bombs, cancelbots, or other harmful computer programming routines.
## Termination or Suspension of Use
Anew reserves the right to terminate or suspend your access to some or all of our online services if you engage in activities that breach these Terms.
Ownership and Intellectual Property
All content on the Platform, including but not limited to software, artwork, text, video, audio, pictures, logos, and other material, is the proprietary property of Anew and its licensors and is protected by copyright and other intellectual property laws. You may not modify, rent, lease, loan, sell, distribute, or create derivative works based on our Platform without express permission from Anew.
Governing Law and Dispute Resolution
These Terms are governed by the substantive laws of the State of Florida. Any disputes related to these Terms or the use of the Platform will be resolved through arbitration under the rules of the American Arbitration Association (AAA) in Florida.
